Four Problem Areas of Borderline Personality Disorder

There are four problem areas of Borderline Personality Disorder in which a person with the disorder struggles: (1) Problems with emotions and moods; (2) Problems with distorted thinking; (3) Problems with self-damaging behaviors; and (4) Problems in relationships.

Core problem areas for those people who have Borderline Personality Disorder are recurrent self-injury and suicidal impulses. A history of childhood trauma (abuse and/or abandonment) is common. Many people who meet the criteria for a diagnosis of Borderline Personality Disorder according to the Diagnostic and Statistical Manual for Mental Disorders (DSM-IV) also have post-traumatic symptoms such as flashbacks and dissociative states (“spacing out”), and panic or anxiety symptoms such as feeling themselves or the outside world to be unreal at times.

Following are the four specific areas in which a person with Borderline Personality Disorder has problems:

• Problems with emotions and moods.
Problems with emotions and moods in people with Borderline Personality Disorder can include very strong emotions which are easily triggered. Some people with Borderline Personality Disorder have learned to cope with this situation by suppressing most of their emotions, but this can be a problem when emotions “break through” anyway. The suppression of emotions may also result in feelings of unreality, “deadness,” and/or boredom. Problems with anger or rage are common. There may be violence or aggressive behavior when angry and there may be blocking of the awareness of anger. Problems with moods often include depression.

• Problems with distorted thinking.
People with Borderline Personality Disorder have distorted thinking. Problems with thinking can include problems in experiencing or with identifying a “normal” identity or sense of self, and difficulty in maintaining a clear sense of their own feelings and thoughts when with other people who have strong views or emotions. They may “absorb” other peoples’ opinions, views, and emotions and adopt them as their own; and/or replace their own original thoughts with those of other people.

• Problems with self-damaging behaviors.
Problems with self-damaging behaviors in people who have Borderline Personality Disorder often include deliberate self-harm and/or suicide attempts in response to their emotional problems. Other similar difficulties may include drug or alcohol abuse, binge eating, risky sexual behavior, and/or gambling problems, which may also be used as ways of attempting to cope with their emotions.

• Problems in relationships.
Problems in relationships for people who have Borderline Personality Disorder can include a great sensitivity to perceived signs of rejection or criticism. They will have strong feelings of love and hate, alternating between the two. Along with this there may be a tendency to not see (or not trust) the positive aspects in a relationship. They may be very needy and dependent – yet push away the very person upon whom they are dependent, or seem to avoid dependency and closeness. They have difficulty in coping with losses and separations. Suppressed emotions can cause difficulties in relationships for people with Borderline Personality Disorder, as can issues surrounding trust.
